*Emergency Info Statement: Should my child become ill or suffer an accident, I hereby authorize Frisco KIDS Shuttle LLC to administer, call for, or secure the necessary care of medical attention as may be deemed necessary by Frisco KIDS Shuttle LLC. I understand that an effort will be made the designated persons if possible, before any action will be taken. I also understand that any expense incurred will be accepted by me.

**In order to reserve a space for your child for enrollment with Frisco KIDS Shuttle LLC, please complete and submit this on-line form along with your child's non-refundable Registration Fee in the amount of Forty-Nine Dollars ($35.00). This annual registration fee is due and payable at the time of enrollment.

***Applicant understands the importance of and assumes responsibility for notifying Frisco KIDS Shuttle LLC of any significant changes in enrollment information such as work and home phone numbers, work location, emergency contact info, authorized pick-up/release persons, medical information, etc, and hereby acknowledges that Frisco Kids Shuttle LLC shall not be held liable for circumstances that are beyond our control (i.e, traffic jams, etc).

****Due to location and space availability, your child's space is not reserved unless this form is reviewed and accepted by a Frisco Kids Shuttle LLC authorized agent via email or telephone.
